Molars carry the highest fees in the D3300 range, so payers scrutinize them most closely. The common failure is a mismatch between the code and the record, as the claim says molar, but the tooth number, canal count, or radiograph does not clearly support it. Sending all three with the first claim resolves most of it.

Retreatment claims must establish that the tooth had prior endodontic therapy and that this work is retreatment, not initial therapy. Payers apply waiting periods to the same tooth, so submit the code with prior treatment history, a pre-operative radiograph, and a narrative explaining necessity.

D3221 is billed when pulpal debridement is performed to relieve acute pain and definitive root canal therapy is not completed the same day. That last part is what payers check. If therapy is completed at the same visit, the debridement is generally considered part of the procedure.

Referral practices bill differently because the patient arrives with a history the office does not own. The limited exam is often the first billable encounter, and it is frequently bundled when treatment occurs the same day. Tooth-level history must be confirmed with the referring office.

CBCT is reimbursed when the image is diagnostically necessary, and the record shows why a conventional radiograph was insufficient. Suspected vertical root fracture, complex anatomy, resorption, and surgical planning are accepted indications. Many plans limit frequency, so verify coverage before the scan.

D3332 applies when therapy is initiated but cannot be completed, typically due to a perforation, a non-negotiable canal, or a tooth requiring extraction. Payers want the clinical reason documented specifically. Because the code pays less than completed therapy, that narrative determines whether it is paid.

Tooth-level history includes prior endodontic therapy for that specific tooth, the date it was performed, and whether a post-and-core or crown is already present. Most billing teams verify benefits at the patient level, but endodontic coverage is determined at the tooth level, and that gap is where retreatment denials arise.
